What is required to be said during the prostration of forgetfulness?

What is the ruling regarding making-up the maghrib or fajr prayer if it has been missed with the passing of sunrise or sunset?

‘Every one [of the congregation] should individually prostrate a prostration [of forgetfulness].’

I have a grandmother who is more than 140 years of age, and is [only] able to move a short distance.

Whoever forgets his errors, the errors of others [then] become more important to him.

I have not heard anything except that I have memorised it, and I have not memorised anything and then forgotten it.
